Employee Receives Multiple Fractures Caught in Mach

At approximately 9:50 a.m. on December 2, 2009, Employee #1, a production supervisor, was attempting to repair the screen of a 220V Everspring printing equipment screening machine, Serial Number 0505001. This machine builds up ceramic capacitor insulation using a screening process that adds the material a single layer at a time. The material is laid down on the plate bed of the machine by an aluminum-framed screen measuring approximately 27-in. square that is operated by a pneumatic piston system. The operator reported to Employee #1 that a tear had developed in the screen. Employee #1 attempted to repair the screen with a piece of tape. With the machine still energized and the screen in the highest position, 15-in. above the plate bed, she leaned over the controls at the front of the machine to access the under side of the screen. She contacted the switch that controls the vertical movement of the screen causing it to lower, pinning her in the machine. Employee #1 was hospitalized with multiple fractures.
