Investigation Summary
At approximately 10:30 p.m. on November 10, 2009, Employee #1 was cleaning a machine (Mitsubishi 185 Summit S2FGR-100) that prints, cuts, glues, and stacks boxes. While cleaning the glue wheel with the wheel rotating, one of his left fingers was amputated. The accident was caused by failure to deenergize and lock out the glue wheel.
