Investigation Summary
At approximately 1:00 p.m. on August 20, 2009, Employee #1, a laborer with JHN Inc. dba Coustic-Glo, was restoring a ceiling in a laboratory room. While restoring the ceiling, she used a five foot high folding scaffold. When the scaffold was moved, Employee #1 lost her balance and fell 3 feet from the scaffold. She struck a table during the fall. She was hospitalized and treated for a shoulder injury and fractured pelvis.
