Investigation Summary

Investigation Nr: 200632339
Event: 12/12/2010
Employee Run over by Truck and Later Dies

On December 12,2010, Employee #1 and five coworkers were sent out to install the existing communication cables from a damaged utility pole to a new pole that had been set earlier. Employee #1 was up on the new pole using his hooks and helping a coworker working from a basket, as they were transferring the communication cables to the good pole. It was determined that a coworker should climb down and put the truck into position to hold on to the broken utility pole until the last cables could be transferred to the new pole. The coworker got into the driver seat, and positioned the truck from the parking lot out into the slow lane. The coworker started backing the truck up to position it to get hold of the damaged pole when Employee #1 walked behind the truck, and the truck ran over him. Employee #1 was transferred to a hospital where his left leg was amputated. He remained in the hospital for several days, Emplyee #1, later passed away due to damage to his lower torso.
