Employee's Finger Is Amputated by Belt and Pulley System

At approximately 11:00 a.m. on May 8, 2009, Employee #1 was cracking corn and making a gluten mix, when the auger became jammed with product and stalled. Employee #1 believed the auger motor had been turned off at the control panel, but the equipment had not been deenergized. In addition, the motor driven belt and pulley system was not guarded. Employee #1 pulled on the belt to reverse the augur and clear the jammed product. When the jam broke free, the belt pulled Employee #1's right hand into the belt and pulley pinch point, resulting in amputation of his fifth digit. Employee #1's third and fourth digits were injured, but surgery was done to save use of the fingers.
