Employee Sustains Injuries by Fireball and Loose Hose

On August 26, 2009, Employee #1 had just completed filling a 3,500-gallon bob tail propane delivery truck from a fixed 18,000-gallon tank. He had detached the vapor recovery hose and the fill hose from the truck and sat it on the ground. The hose had a small amount of propane escaping from it and he tried to completely shut off the valve located on the end of the hose. The vapors from the liquid propane had been blown by the wind into the pilot lights on the liquid propane vaporizer. A flash fire had occurred, and the employee had been engulfed by the fireball. The fill hose was whipping around and spraying liquid propane from the hose. The employee attempted to pick up or turn off the valve, and he was struck in the head by the coupling on the fill hose. He then went to shut off the gas supply with the emergency shut off switch. Employee #1 was flown to Shock Trauma Hospital in Baltimore Maryland for treatment for unspecified injuries and was hospitalized.
