Investigation Summary
On November 28, 2009, Employee #1, a supervisor, went to check on a tarp covered hopper trailer since it had been very windy. He noticed the plastic cover on one trailer had partially blown off and decided to try to retarp the trailer. Using a fixed ladder on one end of the hopper, he climbed up to the first rung. He reached for the cover as a gust of wind blew against the cover. The plastic cover blew back towards the hopper covering the top part of the ladder. As he reached for the ladder with his left hand he could not grab the rung and fell off the ladder. He struck the trailer frame and landed on the ground. He was hospitalized for a collapsed left lung and 3 fractured ribs.
