Investigation Summary

Investigation Nr: 202489811
Event: 11/22/2009
Employee's Fingers Are Fractured When Caught Between Rollers

On November 22, 2009, Employee #1, a sanitation worker at the Blue Diamond Growers' Sacramento facility, was part of a sanitation crew responsible for cleaning equipment associated with the processing of almonds. Just before the accident, he had completed cleaning a dicer, which was equipped with two smooth rollers. To allow easy access to the rollers, the facility's maintenance department had removed metal covers that surround the upper portion of the dicer frame. Employee #1 turned the dicer on via it's disconnect lever and was inspecting his work when he noticed foreign material on a lever just outside the frame of the rollers. With a scrub pad in his right hand, he reached for the lever and his hand became caught between the two rollers. Sensing he could not pull his hand free, he screamed. A coworker hit an emergency switch. Maintenance personnel had to disassemble part of the dicer to free his hand. Employee #1 was transported to a hospital, where he was hospitalized and surgery was performed to insert pins into three fingers that had been fractured. He was released from the hospital during the afternoon of November 23.
