Investigation Summary
On November 5, 2009, Employee #1, a campground maintenance worker, was instructed to remove the pine needles from the sloping metal roof of a small cabin. He decided to first determine how slippery the roof was before he proceeded to rake it. He climbed an extension ladder to access the roof. As he shifted his weight from the ladder to the roof, he slipped and fell 100 inches. He fractured his lumbar vertebrae, but surgery was not required. He was hospitalized for treatment.
