Employee Falls from Roof

On September 23, 2009, Employee #1 was reroofing a two-story residence with Employee #2. The day before the accident, a roofing supply company had stacked OSB sheets (4 ftX8 ft) behind stickers to secure them at different locations on the roof. The roofing supplier also built one unsecured stack by using the top of one secured stack to support one end of the sheets and using the roof's ridge to support the middle of the stack. While installing the OSB sheets, the two roofers picked a sheet from the unsecured stack and set it on the exposed rafters to nail it in place. At some point in the operation, Employee #1 stepped on the edge of the remaining sheets of the unsecured stack, which acted as a teeter-totter with the roof ridge being the fulcrum. The OSB sheets collapsed and slid down the roof, carrying Employee #1 off the edge. Employee #1 fell approximately 12 feet and sustained an unspecified fracture and was hospitalized.
