Employee Is Injured When Excavation Site Collapses

On May 20, 2009, Employee #1 was attempting to locate an underground water pipe that was not in the location indicated on the site plans. An excavator operator expanded the width, length and depth of the excavation site to 15 ft by 30 ft by 15 ft deep. The coworker slowly broaden the site to ensure the water pipe would become visible before he struck it with the excavator's bucket. Unknown to either worker, the water pipe, which was running parallel to the length of the excavation, was located approximately 4.5 ft below the surface and at the spot where Employee #1 was standing. It is believed that the vibration of the excavator and the digging deteriorated the compacted soil around the water pipe causing a catastrophic collapse of the excavation's wall that carried Employee #1 it into the excavation site. He received unspecified injuries that required medical treatment and hospitalization.
