Employee's Foot Is Crushed, Later Dies

On January 7, 2009, Employee #1 was deburring an iron casting, approximately 9-in. by 10-in., weighing between 75 and 113 pounds. The deburring process requires multiple changes of pneumatic tools. Employee #1 completed using one tool and turned to obtain another when the casting rolled off the work table, which measured 34.5-in. by 34.5-in. by 27.5-in. high with a 1.25-in. lip, and struck his left foot. Employee #1 received a severe crush of his foot and was hospitalized. He received initial treatment and follow-up treatment was ongoing. On January 17, 2009 he went to an emergency room with complaints of severe weakness. He was admitted to the intensive care unit, for a pre existing medical condition not related to the accident. He died on January 18, 2009. The cause of death was consistent with massive pulmonary emboli. Employee #1's left foot was swollen and the physician suspected he developed a DVT from this and subsequently had an acute pulmonary emboli.
