Investigation Summary
Employee #1 was using a forklift to hold up tote filled with calcium chloride liquid. Another coworker was gravity filling a day tank with the calcium chloride. Employee #1 got off the forklift and walked in front of it to see what the coworker was doing. As Employee #1 walked around the forklift, observed the plastic tote start to slide off the forks, and attempted to get clear of the falling container. The falling container struck his right foot, resulting in a crushing injury requiring the foot to be amputated.
