Investigation Summary
On October 12, 2010, Employee #1 informed his coworkers that he would see them in the office. The three coworkers assumed he was going to retrieve his lunch from his personal vehicle. Soon thereafter, Employee #1 was found lying on the floor by other coworkers. Employee #1 was in a semi-conscious state and had vomit coming from his mouth. Emergency medical personnel were summoned. Employee #1 was treated at the scene, and then transported to a medical center. He remained in the hospital for nine days until his death on October 20, 2010. The coroner determined that the cause of death was severe head trauma, which was a result of a fall that occurred when he collapsed in the parking structure. Employee#1 sustained a fractured skull, subdural hematoma, and traumatic subarachnoid hemorrhage.
