Employee bumps head on pipe rack and suffers a concussion

At approximately 11:45 am, on May 26, 2010, Employee #1 was checking the galvanized sheet metal on a work truck. Employee #1 was wearing a wide brimmed hat which obstructed the view and subsequently struck their forehead on the metal ladder rack attached to the vehicle. Employee #1 then fell to the ground but did not lose consciousness. Employee #1 was transported to Newport Urgent Care, where they were treated and released. On June 1, 2010, Employee #1 did not feel well and went to Chapman Medical Center in Orange, CA at 5:30pm and received a CT scan. The treating physician informed Employee #1 that he would be staying at the hospital until 11:00pm that evening for observation, however, Employee #1 was then admitted at 11:00pm and received MRI's and EEG's as well. Employee #1 was released from Chapman Medical Center on June 4, 2010 at approximately 6:00pm.
