Investigation Summary
On September 4, 2009, Employee #1 was engaged in the construction of a four story building. He was removing roof skylight opening covers in order to recover the openings in a more secure manner. He picked one end of a piece of plywood and began to walk the plywood on edge. The plywood hung up, causing him to loose his balance. He fell through the opening to a concrete floor, 16 ft. Employee #1 was hospitalized with a fracture to an unspecified body part.
