Employee's Leg Is Caught in Mixer and Is Amputated

On January 8, 2010, at approximately 3:00 pm, a 21 year old male employee was injured while operating a ribbon mixer at the back of a plant. He was mixing dry chemicals to be shipped to a manufacturer for use as absorbent for baby diapers. This operation was performed as needed, and this was the injured employee's first time operating the ribbon mixer. He had been shown the operation one time and then turned loose to operate the machine. The cover of the ribbon mixer had no interlock and/or no non-removable guarding and the cover of the ribbon mixer was not secured. The employee was pulled into the ribbon mixer and became tangled in the mixer spine. Another employee came into the area, saw the employee being pulled around the drum and shut the machine off. The employee was extricated by the Fire Department and rushed to the hospital. As a result of the accident, the employee's leg was amputated and he has little or no use of his left arm.
