Investigation Summary
Employee #1 was performing tank cleaning services adjacent to a waste water treatment construction excavation. Employee #1 crossed over guardrailing to take a short cut back from the truck where the pump was located. The top rail of the wooden guardrailing system broke causing him to fall 22 ft to the floor of the excavation. Employee #1's fall was broken by hitting a protruding pipe on the way down. Employee #1 suffered at least three broken ribs and was checked at the emergency room and released the same day.
