Employee's Fingers Are Caught Between Tubing and Dies

At approximately 11:30 a.m. on August 24, 2009, Employee #1, a laborer, was operating the Roundo section bending machine (Model Number R-3, Serial Number 017508) to feed 1.625 in. steel tubing into the dies. The steel tubing was being fed to form a ring shaped part used to make a bicycle rack. Several fingers on his right hand got caught between the tubing and the rolling dies. His index and middle finger were partially amputated and crushed, and his third finger was fractured. Employee #1 was hospitalized. The employee was wearing gloves at the time of his injury, and had routinely operated the machine for approximately one and a half years. No guarding was installed on the Roundo section bending machine, and the controls were mounted on a moveable pedestal with casters. With the configuration of the machine controls, the operator was able to reach the point of operation with one hand while operating the control with his other hand.
