Employee Falls From Elevation, Later Dies

At approximately 12:00 p.m., on April 16, 2009, Employee #1 was working on the second floor in the warehouse. He was pulling automotive parts and placing them on a cart, so that it could be lifted by forklift truck down to the first floor. On the second floor where the cart had to be placed, was in front of a metal roll up doorway. The metal roll up doorway measured 8 feet wide and it was approximately 16 feet above the concrete floor below. The employee was seriously injured when he fell from that opened doorway that was unprotected, landing on the concrete floor resulting in a severe head injury. NOTE: On May 13, 2009 the Central Office was notified that the victim had passed on Thursday, May 7, 2009.
