Employee Is Killed in Fall from Ladder

On January 5, 2010, Employee #1 and a coworker were performing an electrical punch list of items to be completed. Employee #1 needed to reach previously installed plastic conduits located approximately 15.5 ft above the ground level that were resting horizontally on a metal conduit support. He retrieved a 12 ft fiberglass A-frame ladder and set it up below the area of the conduits that had to be worked on. He climbed the ladder and cut a conduit. He lost his balance and fell approximately 9.5 ft. He struck the top, back, and left side of his head on the concrete floor. His skull was fractured, and he was bleeding profusely. He was transported to a hospital and pronounced dead.
