Employee Is Pulled Into Grinder and Is Killed

At approximately 6:00 p.m. on February 18, 2009, at a remote logging location an employee and a coworker were preparing for end of the workday activities. Both had finished loading the last truck for the day with wood chips conveyed from a Vemeer HG600 horizontal grinder. The coworker was refueling the Barko loader from the west side of the grinder and observed the employee park a skid steer on the east side of the grinder. The coworker noticed that the employee did not return and walked to the other side of the grinder to investigate. The employee had become entangled in the Vemeer's belly and discharge conveyor and the idler roller. The employee was killed.
