Investigation Summary
At approximately 8:15 a.m. on June 24, 2010, Employee #1 and two coworkers were cutting limbs from trees. Employee #1 climbed a gum tree, cutting the first limb and letting it down easy using the hand line. Both his coworkers were cutting other trees. Following this first cut, Employee #1 proceeded to climb to the top of the tree, where he planned to cut another limb. As Employee #1 reached up and tied off, he fell approximately 55 feet to the ground. Both coworkers heard something fall and turned around in the direction of Employee #1, finding him dead. The accident investigation determined that despite Employee #1 wearing the appropriate fall protection, he was unaware that his climb line (fall protection) had crossed paths with the limb lowering line, causing them to burn due to friction. When he reached the top, he leaned back and put his weight on the rope. The rope snapped and Employee #1 fell.
