Investigation Summary

Investigation Nr: 200556090
Event: 06/14/2010
CEILING COLLAPSE

EE #1 and EE #2 engaged in the demolition of a ceiling space area measuring 80 feet by 24 feet by 12 feet, length, width, and height, respectively, at a working height of 16 feet above the ground level for 1.5 days. EE #1 and EE #2 were removing building material by striking the plaster with a (carpenter) hammer to separate the sheetrock from the plaster or to make a hole in the soffit to remove the plaster from the metal frame. When approximately 1/3 of the building material was removed from the ceiling space area, the ceiling collapsed injuring EE #1 , EE #2 and a third employee (EE #3) who was located at the ground level removing debris generated by the ceiling demolition work. EE #1 was treated for a broken bone (arm) and various body bruises related to being struck-by building material. EE #2 injuries were bumps and bruises throughout the body associated with falling from elevation and/or struck-by building material. EE #3 suffered a broken hip and superficial bruises generated by crushing and/or struck-by material on various parts of the body. All three employees were admitted to the hospital for medical treatment. EE #1 and EE #2 were wearing hard hats, steel toe shoes, body harnesses with lanyards that were tied off to the building studs, and safety goggles. EE #3 was wearing a hard hat, safety shoes, safety glasses, and reflective vest. Emergency medical service was onsite within 2-3 minutes of the accident to transport the three employees.
