Investigation Summary
On June 5, 2009, Employee #1, who was changing a rear passenger-side flat tire on the company van in the parking lot of his duty station, was fatally injured when the rear of the van (which was raised using a screw-type scissor jack) fell on him, and caused chest trauma. Employee #1 was rushed to the local hospital where he was later pronounced dead. At the time of the incident Employee #1 was attempting to retrieve the spare tire that was installed underneath the rear area of the van.
