Investigation Summary
On December 24, 2008, Employee #1 was occupying an elevated wood pallet platform on raised forklifts to displace a wedged rollup garage door. He was at an approximate height of 10 ft. As he attempted to displaced the obstructed garage door, he lost his balance and fell. Employee #1 was transported to a medical center, where he died.
