Investigation Summary
On February 18, 2009, an employee was working as a back up operator on a 530 Pepperoni line machine. He had just finished changing the wrapping paper on the machine when he noticed that there was a jam at the knife cutting section of the packaging machine. The employee reached up underneath the discharge guard about 30-in. in an attempt to pull it free from the jammed package, and his fingers came into contact with the cutting blades. The employee removed his hand with two fingers crushed and hanging by just the skin. Emergency services arrived on site, and the employee was transported to the hospital, where he stayed and had two fingers amputated due to injuries caused by the machines cutting blades.
