Employee Is Killed When Caught Between Rollers of Machine

At approximately 7:48 p.m. on February 10, 2009, Employee #1, an adjustor, was asked by a coworker to splice two paper rolls on a Winkler & Dunnebier folding machine that had ripped apart from the rollers. Employee #1 was splicing the paper by manually feeding the paper through the rollers, but he did not release the air on the dancer roller. His neck became caught between the dancer roller and the stationary roller, and he was crushed and killed. Further investigation revealed that coworkers had to release the air to get him out from between the rollers.
