Investigation Summary
At approximately 5:30 a.m. on August 19, 2010, Employee #1 was inside the running machine placing a tape strip over the unit's electronic eye that is fixed upon one of the rotating adhesive drums, in order to override the machine's shutoff function. Employee #1 was holding a part above the drum with his left hand and moving away when his hand slipped causing his third and second fingers to be squeezed between the adhesive coated drum and the compress drum. Employee #1 was taken to a local medical center for treatment.
