Investigation Summary
On June 10, 2010, Employee #1 suffered a hand laceration when it was drawn into the in-running nip point created by material feed rollers of a Pouch Making Machine. This resulted in crushing injuries of digits two and three on the left hand, which were subsequently amputated along with a portion of the web. Employee #1 was adjusting the position of the adhesive tape for which she was not qualified or authorized to do. As reported Employee #1 was waiting for assistance from another co-worker to reload material on the single stage pouch making machine she was assigned to operate.
