Employee Falls in Forklift Accident

On July 2, 2010, Employee #1 was assigned to work at the car lot as a mechanic. Employee #1 borrowed a forklift and platform from the cold storage business across the street to install a sign purchase. The platform consisted of two fruit bins, stacked vertically and a iron framework, that provided lateral stability. Employee #1 was elevated on the platform using the forklift operated by a coworker. During the upward movement, Employee #1 lost his balance, causing him to roll and fall. Employee #1 sustained injury to his back and neck alond with minor contusions.
