Employee Injures Head and Spine in Fall from Ladder

On October 26, 2009, Employee #1, a project superintendent, was working from a fixed ladder, approximately 26 feet from the ground, to close the roof access hatch of one of the buildings he was managing. While Employee #1 was attempting to close the hatch, its handle fell off. He placed the handle in its designated location and tightened the set screw; however, when pulling the hatch closed the handle came off again, causing Employee #1 to fall. He landed on the first landing, about 15 feet below from where he was standing on the ladder and then continued to a concrete floor that was an additional 10 feet below. Employee # was taken to a nearby hospital, where he was hospitalized and treated for severe head and spine injuries. The accident investigation determined that reattachment of the handle required the use of both hands, placing Employee # in an awkward posture and leading to a loss of balance.
