Investigation Summary
At approximately 11:20 p.m. on September 26, 2009, Employee #1, a press operator for a pasta manufacturing company, was sanitizing the dough mixer of a pasta manufacturing machine. While performing this task, the machine was still running and energized, as well as no lockout device was installed. As he was reaching into the mixer, the glove on his right hand became caught and was pulled into the machine. Employee #1 was taken to the Regional Medical Center in Fresno, CA, where he was treated for the partial amputation of his right middle finger.
