Investigation Summary
On August 28, 2009, Employee #1, a machine operator of Strategic Outsourcing Inc., was working as a temporary employee at Soex Textile Recycling USA, LLC clearing out "trash fiber" from underneath a carding machine. He was using a homemade hook made from 0.5 inch rigid conduit to fish the trash fiber out. The accumulated debris had covered the chain and sprocket and Employee #1 could not see it. While removing the trash fiber his thumb was caught in the sprocket. Perimeter guarding without interlocks and no physical barrier was installed due to a previous incident. However, they were not operable during this investigation. Employee #1 was hospitalized and his right thumb was amputated at the distal joint.
