Investigation Summary
On July 29, 2009, Employee #1 was clearing "trash fiber" from underneath a newly commissioned Chinese carding machine with a hook formed from 0.5-in. rigid conduit. The pile of fiber became large enough to cover an unguarded chain and sprocket drive that was mounted on the floor. While removing the fiber pile, Employee #1's right index finger was pull through the chain and sprocket resulting in the amputation of the distal end. Employee #1 was treated for his injury but was not hospitalized.
