Investigation Summary

At approximately 2:30 p.m. on November 19, 2009, Employee #1, a seasonal farm worker, who had been hired and trained the previous day, was picking oranges from the third or fourth rung of a 16-ft aluminum Strathmore orchard pickers' ladder, with rung spacing of 12 inches. A coworker heard Employee #1 fall off of the ladder, and saw him lying face up and motionless on the ground. The coworker and the Field Foreman tried to arouse Employee #1. When he didn't respond to voice and physical stimulus, a CPR certified Field Supervisor attempted to resuscitate him. The Field Foreman called 911, and directed the paramedics to Employee #1, when they arrived. After an unsuccessful hour of administering cardiopulmonary resuscitation, Employee #1 was pronounced dead at the site.
