Employee Is Injured in Fall from Tree

On June 4, 2009, an employee was performing aloft tree work for the first time when the branch he was standing on failed to support him without being tied in causing him to fall approximately twenty feet. The employee was provided a tree worker's saddle and one climbing rope, and denies receiving training for hazards associated with aloft tree work, or safe work procedures. A Crew supervisor claims to have given the employee instructions and training and was directing the tree work. The crew supervisor placed himself on the other side of the tree using a pole chain saw and was not directly supervising the employee.
