Investigation Summary
On May 27, 2009, an employee was accessing the roof of a cold storage facility using two-section Werner aluminum ladder equipped with a three foot hand rail extension. The employee fell while transitioning from the ladder to the roof. The employee was hospitalized and treated for compound fractures to both ankle and fracture of a lumbar vertebra. The employee claimed that he was not carrying anything. The ladder was secured at the top with a clamp and rope, the rungs were not wet or slippery and he was wearing work boots. The extensions allowed the ascender to step through the ladder at the top rather than stepping to the side as customary when the top three rungs extension higher than the wall.
