Investigation Summary

At approximately 11:30 a.m. on February 23, 2009, an employee was removing and replacing combustion-air filters that were mounted on a natural gas fired power generating unit's intake module. The employee had just climbed an approximately 12-ft high fixed ladder inside the module and stepped away from the self-closing gate. He became dizzy and collapsed at the landing. His body's momentum thrust him under the gate, and he fell to the landing below contributing to associated head laceration injuries. The employee was transported to Kern Medical Center for treatment, where he was hospitalized and treated for head laceration.
