Employee Is Injured in Fall Through Barn Skylight

On January 24, 2009, Employee #1, a cattle feeder, was pressure-washing the sloped roof of a dairy milk barn that was being used as a hospital barn for cows. He fell through a fiberglass skylight panel approximately 12 to 14 ft, striking a stanchion prior to landing on the concrete floor. Employee #1 was hospitalized for fractures of his face, nose, spine, and right knee. He could not remember whether he slipped and fell, or stepped on the skylight. The employer failed to guard the skylights, provide fall protection, identify known roof hazards, evaluate operational hazards, and provide training.
