Employee Falls from Truck Lift Gate, Hits Head, Later Dies

On October 1, 2008, Employee #1 was delivering a pallet of paper towels. Employee #1 had parked the truck on a slight incline and was pulling the pallet out onto the lift gate using a pallet jack. Employee #1 apparently lost control, tripped and fell off the 33-in. high gate onto the driveway, striking his head. Employee #1 died on January 2, 2009, from complications due to his head injuries.
