Employee Is Struck by Mast, Asphyxiated and Is Killed

On February 23, 2010, Employee #1 was cleaning a cabinet that was on the forks of a lift truck. He was sitting in the driver's seat of the lift truck. He reached through the mast with his right hand and head, activating the lower lever of the truck. The upper part of the mast came down on his neck, asphyxiating him. Employee #1 was killed.
