Employee Is Electrocuted

At approximately 11:30 a.m. on November 11, 2009, Employee #1 and a coworker were at an oilfield tank farm construction site. The employees had positioned their trucks facing north, with the oilfield construction site to the west side of the trucks and power lines to the east approximately 19 ft away. Employees #1 and the coworker along with employees from the oil field construction company discussed what was to take place, where the tanks needed to be positioned and discussed the power lines on the opposite side of the truck. Employee #1 and the coworker were just completing the off-loading of four fiberglass storage tanks utilizing a truck mounted IMT Articulating Boom Crane, Model Number 16/117 with the assistance of employees from the oilfield construction company. After offloading the last tank, Employee #1, the crane operator, was standing on the ground at the controls of the crane located just behind the drivers' side cab door. Employee #1 swung the crane from west to east with the boom fully extended and came into contact with 7,200 volt power lines located approximately 22 ft in height and approximately 19 ft to the east of the truck. Employee #1 sustained an electrical shock while holding onto the crane controls, and fell to the ground. Employee #1 was transported to the hospital and was pronounced dead by the attending physician at 12:30 p.m. on November 12, 2009.
