Employee Amputates Finger While Cleaning Machine
Employee #1 was cleaning a D2K machine. Employee #1 accidentally placed his hand in a filler tube. The machine actuated, catching Employee #1's left index finger, amputating it up to the first digit. Employee #1 was hospitalized.
