Employee Falls from Ladder Stand, Fractures Leg

At approximately 6:40 a.m. on January 2, 2009, Employee #1, the Shift Mechanic, was changing plugged hoses from the M-line Smoke Stripper Cyclone. The hose run vertically from a height of approximately 10 ft to 3 ft. Employee #1 retrieved a 6-ft mobile ladder stand and placed in front of the unit. Employee #1 locked the unit in place. Employee #1 ascended the unit to an approximate height of 6 ft. The unit slid out, causing Employee #1 and the ladder stand to fall to the epoxy concrete floor. Employee #1 received a fractured left leg and was hospitalized.
