Investigation Summary
At approximately 6:00 a.m. on November 8, 2008, Employee #1 and eight coworkers began a workshift, operating a Spin Line. A Spin Line is a large piece of equipment that is used to spin galvanized metal parts to remove excess zinc. The Spin Line has several work areas that range in height from 11 feet to 3 inches. Employee #1 was designated the lead man of the operation. At about 7:10 a.m., Employee #1 told one of his coworkers that he was going to use the compressed air hose to clean off equipment on the spin line. The coworker left Employee #1 and returned approximately ten minutes later, discovering Employee #1 lying on the floor in a pool of blood from a head injury. The compressed air hose, which ran down the Spin Line equipment from its highest point, was nearby. Emergency services were contacted and Employee #1 was taken to Elmhurst Hospital in DuPage County, where he died.
