Employee Dies in Fall from Cab of Loader Truck

At approximately 11:30 a.m. on April 14, 2009, Employee #1, a 61-year-old Caucasian male, was loading a tractor trailer on a low boy to transfer back to Pennsylvania. While loading the tractor trailer, he fell backward off the cab of the loader truck. He got up and continued to load the low boy trailer. After completing this task, the employee drove to Morris, IL where he fainted and was taken to a local hospital. Later, he was air lifted to a hospital in Peoria, IL, where he died on April 27, 2009. There were no witnesses to the fall; however, one coworker returning from lunch assisted the employee after he fell. The height from the cab to the ground was under 4 ft. Employee #1 had received training on proper load techniques prior to the incident. There was no personal protective equipment required or needed to complete this particular operation. The cause of death was intracerebral hemorrhage (stroke) as per the Peoria County Coroner. There is no evidence of intracranial or cerebral trauma that initiated the cerebral hemorrhage.
