Employee Amputates Finger While Using Panel Saw

At approximately 7:15 p.m. on July 27, 2009, Employee #1 was cutting scrap sheets of 0.125-in. by 8-ft by 4-ft sheets of plastic with a panel saw. A small piece of the plastic measuring approximately 1-in. wide by 12-in. long, hung up behind the saw blade and in the throat of the vacuum waste hose. Employee #1 reached out to clear the piece, touched the spinning 16-in. blade and amputated his right index finger. Employee #1 was hospitalized.
