Employee Amputates Right Thumb Moving Mobile Bar

At approximately 8:30 p.m. on October 12, 2012, Employee #1, with Four Seasons Hotel, San Francisco, was setting up a bar and assisting the Bartender during a banquet event. When the banquet event was over and the mobile bar was being moved back to its storage area by two Bussers, Employee #1's right thumb caught between the rolling (mobile) bar and the panic door lever. He suffered a crushing amputation of his right thumb fingertip and was transported to the hospital by paramedics. The severed portion could not be replanted but was used for skin grafting of the wound. The cause of the accident was lack of handles on both ends of the mobile bar and possibly speed. No serious-accident related violation was issued. Employer failed to report the serious injury to the District Office and was cited accordingly.
