Investigation Summary
At approximately 10:00 a.m. on January 19, 2011, Employee #1 was working alone, as usual in the Gravity Thickening Belt Building. Employee #1 was filling a 400-gallon above ground tank with a proper mixture of solvent and water. Employee #1 used a rolling scaffold to gain access to the fill port of the tank. Employee #1 rolled the scaffold into place, locked the wheels and ascended the attached ladder. Employee #1 stepped off the scaffold onto the top of the tank and poured approximately one gallon of the solvent into the tank. When finished, Employee #1 turned around to step back onto the scaffold but had turned the opposite way of entry ending up facing away from his intended position. While turning back around to correct his position, Employee#1 hit his knee on the mixer motor attached to the top of the tank. At this time Employee #1 thought he had merely bumped his knee as the skin was not broken and did not have any other adverse effects until later that evening. Employee #1 reported this incident to the Employer on January 20th. Employee #1 saw a doctor on, and was admitted to, Kaiser Hospital in San Francisco on January 25, 2011. Employee #1 was released from the hospital on January 27, 2011 and placed on a modified work schedule until February 3, 2011.
